About 5 million people enrolled in Affordable Care Act health plans and then didn’t pay their first premium — a drop-off rate sharper than anything recorded in recent years. Trump administration officials say fraud is driving the numbers. Their argument: people are being signed up without their knowledge or consent, inflating enrollment figures and then collapsing when payment comes due.
Health policy experts don’t buy that. They say the culprit is cost — that premiums climbed after Congress let enhanced subsidies expire, leaving lower-income enrollees with bills they couldn’t absorb. When the math stopped working, people walked.
The distinction matters. A lot. If fraud is the problem, the fix is tighter enrollment verification. If cost is the problem, the fix is restoring subsidies — something the current administration and Republican congressional majorities have shown no appetite for.
The drop-off is steep enough that researchers are treating it as an outlier. In past years, a share of enrollees always failed to pay their first premium — that’s normal churn in any large insurance program. But the scale of this year’s non-payment is outside the historical range, which is why it’s drawing attention from both sides of the debate.
Neither explanation has been settled. Federal health officials haven’t released detailed data that would let outside researchers independently test the fraud claim — an absence critics say is telling. Health policy analysts, meanwhile, are working from premium data and enrollment trends to make their case that affordability is what broke down.
The unresolved question is whether Congress moves at all. Enhanced ACA subsidies that held premiums down for millions of households lapsed, and there’s been no sign of a deal to bring them back.
